CLRC Learner Practice Request Form

This form is intended for University of Saskatchewan (USask) learners seeking CLRC space for additional independent practice opportunities during their free time. Those requesting space through this form should familiarize themselves with the CLRC Learner Practice Policy before making a request.

USask health science programs and USask-affiliated units wishing to schedule academic programming within CLRC spaces should use the CLRC Space Booking Form.

External groups seeking space for simulation events, conferences and other sessions (e.g., licensing exams), are advised to book CLRC space via USask Event Registration and Room and Space Booking.

In the form, indicate that the event being requested will be in the USask Clinical Learning Resource Centre.

Standard hours of operation are 8:30 am to 4:30 pm, Monday through Friday. Evening and weekend operations vary based on teaching and learning requirements.

Training is required before using any CLRC equipment and must be renewed annually.

Cancellations must be sent to clrc.scheduling@usask.ca a minimum of one business day before a requested event.

Any events cancelled less than one business day in advance may have associated fee assigned to them.

In this form, student groups will need to identify whether or not they are an officially ratified student group.

NOTE: Submissions through this form are not reviewed by the CLRC on evenings, weekends, or holidays.

Please refer to the CLRC Learner Practice Policy to review CLRC regular hours and after hours times. No practice times are available on statutory holidays, weekends, or when the university is closed (e.g. the winter holiday break)

I acknowledge that I have reviewed and will abide by the requirements outlined in the CLRC Learner Practice Policy and that I have (or will have) received proper training in advance of my requested practice session. I also acknowledge and understand that all rooms must be left exactly as they were set up at the beginning of my practice session and must be ready for courses the next morning.
