Key and Access Card Request/Cancellation Form

Key and access card requests/cancellations for the Health Sciences Building must be submitted through your authorized key custodian through the form below. 

Any disruptions or difficulties with access devices should be reported to the respective key custodian first. All issues must be submitted through healthsciences.keys@usask.ca.

Please enter the required information for each requested/cancelled key. Be sure to include the quantity, key series and room number for each key. Enter the key series for standard/metal keys (if known). The key series for electronic access requests is PROX.

Key custodian information
