CLRC Equipment Loan Request Form

This form is intended for University of Saskatchewan (USask) learners and programs seeking CLRC equipment for course sessions, learner practice, or other educational activities for a designated amount of time.

Those requesting equipment through this form should familiarize themselves with the CLRC Equipment Loan Request Policy before making a request.

Standard hours of operation are 8:30 am to 4:30 pm, Monday through Friday. Evening and weekend operations vary based on teaching and learning requirements.

Training is required for some CLRC equipment and must be renewed annually.

NOTE: Submissions through this form are not reviewed by the CLRC on evenings, weekends, or holidays. To learn more equipment loan requests or training requirements, please email clrc.scheduling@usask.ca.

* Requested loan period

Unless previously arranged, requested equipment can be loaned for a period not exceeding five (5) business days. Equipment will not be loaned during weekends, unless previously arranged. Equipment pickups and returns are unavailable on statutory holidays, weekends, or when the university is closed (e.g. the winter holiday break).

All equipment must be returned to the CLRC reception area no later than 4:30 pm on the date scheduled for return.

I acknowledge that I have reviewed and will abide by the requirements outlined in the CLRC Equipment Loan Request Policy and that, where applicable, any person(s) using equipment loaned by the CLRC through this request will have received proper equipment training in advance of using the equipment.
